How do I add a form to a page?

1. Go to "Pages" and select "Add New"Go to "Pages" and select "Add New"

2. Add the title of the page and select "Add Form" 

3. Select "Existing Forms" to find the one you created and then select "Insert into post" under the form you want to insert

4. Select "Publish" to publish your page.
